If you had an IEP or 504 Plan in high school, have a physical, psychological, or learning disability, or are deaf or hard of hearing, we encourage you to connect with our office. All contact with Disability Services is confidential and will not appear in your official college records.

Step 1:  Complete the intake form

To begin the process, please complete our Disability Services application form.

• Log into MyGCC
• Click the green Submit a Form button on the left menu
• Select Disability Services Application Frm
• You can submit supporting documentation along with the application form

If you are not able to log into myGCC yet, please email us and we will email you the form instead. Use our secure upload link to submit the form and supporting documents.

The information you provide on this form helps us better understand your needs and ensures important details aren’t overlooked during our conversation.

Step 2:  Submit documentation related to your disability

Please provide your most recent disability documentation before your appointment, if possible. This allows our staff time to review your materials and prepare for your meeting.

If you do not currently have supporting documentation, you may still be eligible for accommodations based on the information you share with us.

All documentation received by Disability Services is confidential and is held in accordance with FERPA regulations.

Here are some examples of documentation that can support your request for accommodations:

• Educational and Psychological evaluations
• Individual Educational Plan (IEP) or 504 Plan
• A letter or report from a healthcare provider clearly stating your diagnosis
• GCC’s Disability Verification Form. Email us for a copy

Step 3:  Schedule a one-hour intake appointment

Once we have received and processed your application form, we will email you a link to schedule an appointment.

We strongly encourage you to connect with Disability Services before the semester begins to ensure that your accommodations are in place when classes start. Some accommodations—such as sign language interpreters, captioning services, or braille materials—require advance planning and may take time to coordinate.
